- Your doctor asked for you to be enrolled because you have one or more chronic health conditions that we can help to support.
Will there be additional costs?
- Our services are covered by most insurance plans and most or all costs are covered like your doctor visits. You may have a co-payment or deductible depending on coverage.
- We can contact your carrier to confirm what, if any, out-of-pocket costs you may be responsible for.
What is expected of me to be involved?
- To be enrolled in a remote monitoring program, we expect you to be taking measures at least every other day.
- It is important that our nurses are able to see your vitals in realtime to understand the best ways they can help.
Why is my doctor using Welby?
- We partner with primary care providers to help them access additional clinical resources to help provide support to patients at home.
- The work we do at Welby is directly in conjunction with your physician, and we collaborate closely on care planning and recommended activities.
How does this work exactly?
- We set up an initial consultation with one of our registered nurses to evaluate needs.
- If we can provide support, we will send a monitor to evaluate things like blood pressure, weight, or blood glucose.
- We can then monitor those results in real-time and work with you on strategies to help improve your health with a variety of coaching tools.
