Why was I selected to participate in this program?
Community Physicians asked for you to be enrolled because you have one or more chronic health conditions that we can help to support.
Will there be additional costs?
- Our services are covered by most insurance plans and most or all costs are covered like your doctor visits. You may have a co-payment or deductible depending on coverage.
- We can contact your carrier to confirm what, if any, out-of-pocket costs you may be responsible for.
How is my doctor connected to Welby services?
In addition to your Welby nurse case manager, your provider will have access to all of you readings. Furthermore, your Welby nurse stays in communication with your provider so they stay up-to-date with your health.
Why is Community Physicians using Welby?
- We have partnered with Community Physicians to help provide support to their patients at home.
- The work we do at Welby is directly in conjunction with Community Physicians, and we collaborate closely on care planning and recommended activities.
What is expected of me to be involved?
- To be enrolled in a remote monitoring program, we expect you to be taking measures at least every other day.
- It is important that our nurses are able to see your vitals in realtime to understand the best ways they can help.
How does this work exactly?
- We set up an initial consultation with one of our registered nurses to evaluate needs.
- If we can provide support, we will send a monitor to evaluate things like blood pressure, weight, or blood glucose.
- We can then monitor those results in real-time and work with you on strategies to help improve your health with a variety of coaching tools.
Can I stop using Welby after I sign up?
There are no long-term commitments when signing up for Welby Health. You may unenroll at anytime without penalty.
Do I need to know how to use technology to use Welby monitoring?
Certainly not. Our user-friendly connected health devices are very simple. The devices are cellular-enabled and do not need to be connected to Wifi or Bluetooth.
