Please read these Terms of Use carefully before enrolling in Welby Health's RPM and/or CCM programs.

By clicking "Complete Enrollment" or enrolling in the program, you acknowledge that you have read, understood, and agree to be bound by these Terms of Use. You may not participate in the programs if you disagree with these terms.

• Program Description
  Welby Health's RPM and CCM programs utilize technology and communication to help manage your health conditions. This may include remote health data monitoring, regular communication with your care team, and personalized support to help you reach your health goals.
  
  
• Voluntary Participation and Opt-Out
  Your participation in these programs is voluntary. You may withdraw from the program at any time at the beginning of any calendar month by notifying Welby Health in writing.
  
  
• Financial Responsibility
  You acknowledge that depending on your insurance coverage, you may be responsible for a cost-share or co-payment. Welby Health will make reasonable efforts to inform you of any potential costs associated with the program.
  
  
• Communication Consent
  You consent to receive text messages and emails from your Welby Health care team regarding your participation in the program. These communications may include reminders, updates, and requests for information related to your health.
  
  
• No 24/7 Monitoring
  You understand that these programs do not provide 24/7 monitoring. If you are experiencing a medical emergency, please seek immediate medical attention by calling 911 or visiting the nearest emergency room.
  
  
• Equipment Use and Return
  You agree not to tamper with or modify any equipment Welby Health provides. You also agree to return all program-related hardware to Welby Health upon completion or withdrawal from the program.
  
  
• Device and Data Exclusivity
  You agree that you will not share your monitoring device with anyone else and will be the only individual sending data readings to Welby Health.
  
  
• Data Ownership and Usage
  You acknowledge that Welby Health will store all of your health data in a HIPAA-compliant manner. You grant Welby Health the right to use your de-identified data to improve our clinical models and other purposes permitted by HIPAA. All data received will become the property of Welby Health, but you may request a copy of your data for your records.
  
  
• Program Compliance
  You agree to make every reasonable effort to comply with the requirements of the RPM and CCM programs. This includes taking and transmitting readings as instructed (generally 16 unique days of readings each month) and actively participating in developing and implementing your care plan.
  
  
• Changes to Terms of Use
  Welby Health reserves the right to modify these Terms of Use at any time. Any changes will be effective immediately upon posting the updated Terms of Use on the Welby² Health website. Your continued participation in the program after posting any changes constitutes your acceptance of those changes.
